During your first contact you will need to complete the following
forms.
· Client Information Sheet
· Policies and Procedures (for your records)
· Child Intake Questionnaire (for clients under 18)
·
Adult Intake Questionnaire (for clients 18 and over)
If you would like your results shared with other persons
or professionals (e.g. family member, schools, or physician), you will fill out a Release of Information form as well.
Please be sure to arrive at least 20 minutes in advance of your appointment time to allow ample time to complete this important
information.
What to bring to your
first appointment
· Copies
of Records (Medical/Psychological/School)
· Report cards,
test scores, test reports or any other records pertinent to ADHD or learning
disabilities are very
helpful to us in making an accurate assessment.
Please bring photocopies of these items rather than
original copies. We want to guard against the possibility of losing your original copies. If you prefer, you may send copies of records in advance of your initial consultation. This will facilitate the history-taking phase and facilitate
the process of making recommendations and beginning treatment.
